Reporting lost or stolen passports

Any passport which is either lost or stolen should be reported to the police in order that appropriate investigations can be undertaken.

 

Any foreign national reporting the loss or theft of their passport or national identity card must also report that loss/theft to their embassy/consulate in the UK without delay.

 

Any person who has lost their UK passport or had it stolen is also required to notify the UK Passport Service (UKPS) of that loss/theft as soon as possible.

 

Notification to the UK Passport Service should be made using a form LS01, which can be found below.

 

Copies can also be obtained from the following outlets:

  • The Passport Advice line on 0870 521 0410
  • The UK Passport Service at www.passport.gov.uk
  • Your local passport office
  • Your local police station
  • Selected post offices and branches of Worldchoice travel agents that offer the passport application ?Check & Send? service

Guidance notes on completion of form LS01 are included at the top of that document.

 

The document must be printed and forwarded to the UKPS in the size and format presented to you and should not be altered. It is recommended that a blank form be printed for completion by hand.

 

On completion it should be forwarded to the UKPS, details of which are included on the form.
