The subject access provision of the Data Protection Act gives an individual the right to have a copy of any data held on them. Additionally it provides a right
to have inaccurate data rectified, blocked, erased or destroyed.
Should you wish to know what information (if any) the Avon and Somerset Constabulary holds about you, or you would like confirmation that you do, or do not have
a prosecution/conviction history, you must complete a subject access form.
Payment of a £10.00 fee and two forms of identification must accompany a completed application form. The identification must, between them, be capable of showing name,
date of birth and current address. Please do not send original documents, only photocopies are required.
Application forms (form 462) are available at the front office of any of our police stations; alternatively you can download a copy of the form below.
Please note: Although we provide the document download in three formats (Flash, PDF and Text only) the Text only version should be avoided in this instance as it does not transfer well to print.
