Civil claims guidance for regulatory bodies and representatives.

You should only complete the online application form if you are requesting material to carry out statutory functions relating to fitness to practice legal proceedings.

Once we have received the non-refundable initial payment, we will provide a cost for the following, if available:

  • Police report summary
  • MG5 Summary of incident – redacted
  • Victim/Witness statements – if consent is provided
  • Victim/Witness statements – redacted if consent is not provided
  • Victims contemporaneous notes – if consent is provided

If you are requesting a police report and the investigation is still ongoing, you will only be provided with limited information.

The information will be provided to you once final payment has been received.  You must make sure the material is accessed within 4 weeks.

Court Order

If you want to request material not mentioned above, you will need to provide us with a court order.

You will also need to provide a court order when requesting:

  • third party records of taped interviews, contemporaneous written records of interviews, summaries of taped interviews and statements under caution
  • un-redacted copies of any of the information above
  • information held in police prosecution files
